Job Summary:
As a Hospice RN Case Manager, you'll coordinate and deliver skilled nursing care to patients in their homes. You'll play a vital role in developing and managing individualized care plans, working with the patient, their family, and the IDG/IDT to ensure comfort, dignity, and support during every stage of care.
Key Responsibilities:
- Complete initial and ongoing comprehensive assessments of patients and families.
- Develop and implement individualized plans of care in coordination with the interdisciplinary team.
- Provide education on disease progression, symptom management, end-of-life care, and self-care.
- Administer preventive and palliative nursing procedures to ensure patient comfort and symptom control.
- Supervise and provide clinical direction to Hospice Aides and LPN/LVNs.
- Maintain accurate, timely documentation to ensure continuity of care and regulatory compliance.
- Communicate proactively with physicians and other team members regarding changes in patient condition.
- Participate in on-call rotation and emergency PRN coverage as needed.
- Promote infection control and safety measures during all care interactions.
- Actively participate in QAPI initiatives and agency training to support quality standards.
Qualifications:
- Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s degree in Nursing).
- Current RN license in Texas (or Compact State License, per Board of Nursing requirements).
- Valid driver’s license and reliable transportation.
- 1+ year clinical RN experience required; hospice experience strongly preferred.
- Strong documentation, organizational, and communication skills.
- WellSky/Kinnser experience preferred.
- Must be able to lift up to 50 lbs and work in home environments with varying conditions.
- Spanish-speaking is a plus, as many of our patients and families are most comfortable communicating in Spanish.
